Through its awards, APTA Idaho honors individuals whose contributions to the profession and the Chapter are outstanding and substantial. For individual members so honored, Chapter awards represent a high level of peer recognition.
J. Perry Silver Award For outstanding service to physical therapy
This award is not given out every year.
HISTORY
Established in 1981 to recognize excellence in clinical practice, dedication to patients, to the profession, and to sharing knowledge with others. The late J. Perry Silver was an outstanding physical therapist in Idaho. His example of clinical excellence and service to his profession has been an inspiration to his colleagues.
PURPOSE
To acknowledge the achievements and contributions of Idaho’s outstanding physical therapists.
ELIGIBILITY
- An active member of APTA Idaho.
- A person who has engaged in physical therapy practice for at least 15 years.
- A person whose allegiance has been to the patient and the profession.
- A person who has participated actively in Chapter activities, giving generously and willingly of time and effort on behalf of the profession.
- A person who has made contributions to physical therapy in general by dissemination of useful and fundamentally sound information through such media as: publications, lectures or films, leading workshops or in radio or television programs.
NOMINATIONS SHOULD INCLUDE
- History of the individual’s professional clinical experience, description of job responsibilities, and examples of outstanding clinical contributions.
- Evidence of the individual’s professional allegiance to his/her patients as demonstrated by honors and awards, record of community activities on behalf of the physically challenged, letters from former patients, students, hospital administrators, physicians, supervisors, employees, etc.
- A record of service to APTA Idaho.
- A list of efforts and their outcomes to disseminate useful and fundamentally sound physical therapy information.

PT or PTA of the Year Award
One award is awarded to a Physical Therapist OR Physical Therapist Assistant per year.
PURPOSE
To recognize outstanding clinical practice Physical Therapists or Physical Therapist Assistants in the State of Idaho.
CRITERIA FOR SELECTION
- Each PT or PTA nominee must have a license or otherwise qualified for at least 5 years.
- The PT or PTA must be an active member in the APTA and APTA Idaho currently.
- PT or PTA must have attributes which have affected quality of patient/client care.
- Contributed to overall delivery of Physical Therapy services to patient level and within state or national level of APTA or APTA Idaho.
- Serves community through activities that enhance quality of life and function of public.
- Promotes ethical standards and professional conduct among peers , patients and students.
- Pursues professional development through continuing education (workshops in-services, etc.) according to state requirements.
- PT or PTA can be re-nominated for the award after a period of 3 years after receiving the award.
PROCEDURE FOR NOMINATION:
- Nominations may be submitted by any current member of the association.
- Nominations must be submitted in writing to include Name, Contact information and a written statement of why the individual is a worthy candidate. The written statement must include: – Name of member making nomination
– APTA number of member making nomination
– Name of person being nominated
– Description of why this person should receive the Jill Cooper Award. - Completed nominations must be received by August 31st for consideration for the Annual Meeting that year.
- Letters of support can be submitted from PTs, PTAs, patients, or administration having knowledge of nominees service record and character.

Jill Cooper Friend of Physical Therapy Award
PURPOSE
To recognize a person, who by his or her actions, has indicated exceptional support of physical therapy.

- Has supported the profession in the community.
- Has contributed to the professionalism of physical therapy.
- Has portrayed the profession in a positive way

Outstanding Physical Therapist and/or Physical Therapist Assistant Student Award
HISTORY
Established in 1993 to give support to physical therapy and physical therapy assistant students and to encourage active participation of student members in APTA Idaho and APTA.
PURPOSE
To acknowledge students who are going into their final year of study, who excel academically and clinically, and who have contributed time and effort to APTA Idaho or APTA. [For example a student nominated in August of 2008 will graduate from the program in May of 2009]
ELIGIBILITY
Nominees must be active student members of APTA, currently enrolled in a PT or PTA program and have a permanent address in the state of Idaho. Preference for this award is given to students who either remain in or return to Idaho and who actively participate in the APTA Idaho.
PROCEDURE FOR NOMINATION
Nominations for the awards may be submitted by non-members and members of APTA Idaho, including faculty members or clinical instructors, and should be submitted to APTA Idaho using the form below by August 1 of the calendar year in which the award is to be given. APTA Idaho Executive Committee will recommend recipients to the Board of Directors for approval at the Association’s Spring Board of Directors Meeting.
NOMINATIONS SHOULD INCLUDE
- Letter of nomination.
- Grade point average and ranking in class.
- At least two letters of support, including academic advisor or clinical instructor.
- Listing of APTA Idaho or APTA activities and reason(s) why the student is deserving of the nomination.
- Evidence of leadership in the academic program.
